Safety incidents, quality failures, and flagged low scores, logged when they happen. Counted per site and per month, they give a safety or quality goal a rate to bring down.

How long each flagged issue stayed open, and who closed it, so fixing problems fast can be a goal of its own.
Task and checklist completion
Whether an assigned checklist, work instruction, or training item was completed and on time, tied to the worker or shift it was assigned to.
